Hollywood has lost another icon. Patty Duke passed away early Tuesday morning, Access Hollywood has confirmed. She was 69.

According to her rep, the actress died from sepsis after suffering a ruptured intestine.

A statement was issued following the sad news, calling the star “a wife, a mother, a grandmother, a mental health advocate and a cultural icon.”

“She will be greatly missed,” the statement continued.

Duke was best known for her dual role as identical cousins on the 1960s sitcom “The Patty Duke Show,” and her Academy Award-winning turn as Helen Keller in “The Miracle Worker.”

She is survived by her husband, Michael Pearce, and three sons — Kevin Pearce, Mackenzie Astin and Sean Astin.

Duke and Pearce celebrated their 30th anniversary on March 15, a milestone which the actress shared on Twitter.
